I just posted this in response to another question, have you heard of www.goalbookapp.com? They have a great action plan/target area template for students based on need, they also offer a free 30-day account trial.
One thing that has worked for me is to look at where the student is in relation to where I'd like them to be by the end of the school year, then move backwards in reasonable chunks (for example if a student is reading at an F&P level C and the grade level expectation end of year is H, then I can decide where our goal should be in 2 months, etc.).
I would also recommend working with the special education specialist/reading specialist, or even the teachers at the grade level just behind and ahead of you at your school to come up with a specific learning plan.
