What do you do about frequent bathroom users in class?

Our school policy has the 10/10 rule. The students cannot go until 10 minutes after the first bell and they cannot go if there is 10 minutes left in the class. My question is-- what do you do for those repeat offenders that go every class? Even if there is a tracking system of some sort, and I know they are going because they're bored or want to text/call someone-- how could I handle that without Impeding on their human right?
In 2 years they'll be making a career decision for the rest of their life but they still have to ask to go to the restroom because when you give teenagers and inch-- they take a mile.

      I have an issue with tardiness in my classes... one thing I've done is tell my kids that if they are late, they cannot go to the restroom. I figure if you're going to be late, might as well be late an extra minute and use the restroom; you are not going to waste anymore class time than you already have.

        I started the semester with a 5 pass rule. They got 5 free passes to go to the bathroom for the whole semester. They got tickets and it was their responsibility to not lose them. This was still pretty generous in my opinion but gave them some leeway for times when they really needed to go. I talked about the point of breaks and lunch and how they had to think ahead and about their needs and that honestly more than 5 "emergencies" at their age was not very credible. Hope that helps!

          Each class has Exeats which each student that wants to use the restroom takes along , no other student in the class visits until the previous student comes back. this reduces loitering around the school or evading classes.
          talks are given ocassionally that its part of home and toilet trainings to be able to restrain oneself from constant visits to the restroom, however there is a student that has been discovered with a bladder problem, a teacher is assigned to remind her to visit the restroom every ten minutes the parent's attention has been drawn and she is being attended to medicallyin the hospital, , her situation indirectly made others appreciate holding on till break and lunch periods.
