I like using class dojo! It gives immediate feed back to parents and students.
I think it comes from being very intentional and well planned. For each and every transition, procedure - model it - collectly develop "look forms" - post the "look fors" and refer back to those before engaging in that procedure or transition. Thus, involve students in developing norms/rules/expectations - post them visually- and constantly refer back to them (before you even see the unintentional behaviors happen).
